

ASQ-3 Developmental Screening and
ASQ:SE-2 Social-Emotional Screening
For our current families:
Avenue Preschool offers parents of currently enrolled children the opportunity to participate in an annual developmental screening. The developmental screening tools we offer are The Ages and Stages-3 (ASQ-3) and The Ages and Stages:SE-2 (ASQ:SE-2) Questionnaires. These screening tools have been approved by the State of Delaware, Department of Education.
Parents or primary caregivers will complete the screening tool through an online portal from the comfort of their own home. Information on how to access the screening portal will be sent home with your enrolled child.
Please refer to your Avenue Preschool Family Handbook, and the information sent home with your child for further details about the developmental screening tool and process.

Child Health Appraisal Form
to access the child health appraisal. This form is required to be completed and submitted to Avenue Preschool upon enrollment of your child, as per State of Delaware regulations.
Authorized Pick-Up/Sign-Out Form

Child Developmental Milestones
Developmental milestones are behaviors or physical skills seen in infants and children as they grow and develop. Rolling over, crawling, walking, talking and problem solving are all considered milestones. Developmental milestones are different for each age range, and there is a typical range in which a child may reach a particular milestone.

Delaware Stars
Avenue Preschool participates in Delaware Stars for Early Success. This is our state's Quality Rating and Improvement System for early education programs, and it is administered by the Delaware Department of Education. As part of Delaware Stars, Avenue Preschool strives to meet certain levels of quality in these key areas: Family/Community Partnerships; Staff Qualifications/Professional Development; Learning Environment/Curriculum; Management/Administration.
